Individuals that are eligible will meet the following guidelines:

•  Live within Daviess County, KY

•  Receive no healthcare insurance from any source

•  Have an annual income no greater than 150% Federal Poverty Level

          •  $14,355/ one person

          •  $29,025/ family of four

•  Additional family members- $4,890

If eligible for Medicaid, Medicare or other programs, applicants will be referred to appropriate program.
