• If an individual is ineligible for DC-CAP. They will be referred to     other programs and/or clinics

•  Prescription Assistance

•  Transportation Assistance

•  Interpreter Assistance

•  Case management – Provide appointment reminders and Follow ups

•  Link patients with Participating Provider as needed
